Patient Information

Thank you for choosing Mountain View Family Medicine for your healthcare needs. Our desire is that you are able to live life with good health and happiness. We work extremely hard to partner with patients each day to provide this experience. We are pleased to be part of your personal healthcare team.

Make an Appointment

To request an appointment, call us at (865) 579-5080 Monday-Thursday, 8:30 a.m. until 4 p.m. Please call at least 24 hours in advance for appointment cancellations or rescheduling.

Patient Registration

On your first visit, you will be asked to fill out registration forms. You may choose to pre-register by downloading, printing and completing the forms below. Just bring them to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.

Patient Reminders 

We ask that you please arrive 10-15 minutes prior to your scheduled appointment. Please bring the following:

  • Your current health insurance information – to ensure accurate and appropriate billing of your health care services.
  • Your current Photo ID
  • Any medications you are currently taking in their original containers including prescription and over-the-counter medications

Cancellation Policy

When you schedule an appointment, we block a provider’s time for you and therefore cannot schedule other patients at that time. We must efficiently schedule our providers to be able to provide services to you at a reasonable cost. We kindly ask that you give us at least 24 hours notice if you need to cancel an appointment for any reason. Should you cancel and fail to give us proper advance notice on three occasions, we reserve the right to ask you to transfer your care to another practice.


Mountain View Family Medicine participates with Medicare and most major HMO and PPO insurance plans. As a courtesy, we will file all claims with your insurance company. If you have questions regarding your insurance coverage, please contact us at (865) 579-5080. If your insurance coverage includes co-pays we will request payment of these at the time of your visit, as well as any outstanding balances.
